A Minimally Invasive Approach to the Lumbar Neural Foramen and Extraforaminal Compartment: Modified Surgical Technique
نویسنده
چکیده
Surgical approaches to the neural foramen and the lateral extraforaminal space in the lumbar spine are required to treat far lateral disc herniations and shwannomas. The MetRx tubular retractor system facilitates a minimally invasive technique that directly targets the region delineated by Kambin’s triangle. This study presents technical modifications, described in a step-wise fashion, to previously published approaches. We reviewed the charts of 47 patients that underwent surgery for pathology located in Kambin’s Triangle using the modified technique described here. Important technical adjustments include docking on the pars interarticularis, minimal shaving of the lateral pars and inferior articulating process, and palpation of the pedicle as a reference point for opening the ligamentumflavum and locating the exiting nerve. Potential pitfalls and strategies to avoid them are also noted. All 47 patients reported a successful outcome with the procedure, there were no complications or revisions, and all were discharged home within 24 hours of the procedure. Key elements of the procedure were highlighted with supporting illustrations and intra-operative imaging. The minimally-invasive approach performed through MetRx tubular retractors is a safe and successful surgical method for treating pathology localized in the neural foramen and lateral extraforaminal space between L1L5 vertebrae. *Corresponding author: Michael S. Prime Incision and Modified Moving Window: A Minimally Invasive Access for Breast Cancer Surgical Treatment
BACKGROUND Conservative surgical treatment has been the treatment of choice for early breast cancer. It allows feasible oncological treatment with a satisfactory cosmetic approach and fast recovery. However, in some cases mastectomy is necessary. This study proposes a surgical approach with only one surgical access through the same incision, which is in line with precepts mentioned abov...
متن کاملMinimally Invasive Approach For Extraforaminal Synovial Cyst L5-S1.
Symptoms from synovial cysts are produced by neural compression in the spinal canal or the foramen. Few cases of extraforaminal synovial cyst have been published in the literature. This is a case report of a 65-year-old female who presented with a three-month history of sciatic pain and no relief with conservative treatment. MRI showed a left-sided extraforaminal synovial cyst at L5-S1 with com...

Background and Aim: Degenerative lumbar scoliosis is a spinal deformity resulting from advanced disc degeneration and facet arthropathy. Given the inconclusive available literature and lack of high-quality data supporting the role of minimally invasive surgical management of degenerative lumbar scoliosis, this review intends to highlight and compare the various viable minimally invasive surgica...
متن کاملMinimally Invasive Surgical Approach to Distal Fibula Fractures: A Technique Tip
Wound complications following ankle fracture surgery are a major concern. Through the use of minimally invasive surgical techniques some of these complications can be mitigated. Recent investigations have reported on percutaneous fixation of distal fibula fractures demonstrating similar radiographic and functional outcomes to traditional open approaches. The purpose of this manuscript is to ...

PURPOSE This study aims to investigate the feasibility of a novel lumbar approach named extraforaminal lumbar interbody fusion (ELIF), a newly emerging minimally invasive technique for treating degenerative lumbar disorders, using a digitalized simulation and a cadaveric study. METHODS The ELIF surgical procedure was simulated using the Mimics surgical simulator and included dissection of the...
